Your First Visit to see a Retina Specialist?
If this is your first time to see Dr. Iyer, your appointment will be detailed and may take up to two hours. Your eye(s) will probably be dilated so you may want to bring a pair of comfortable sun glasses. The office will supply you with some temporary glasses if you forget. You may wish to come 15 to 30 minutes early to fill out the appropriate paper work.

• List of medications
• List of doctors you are seeing and the referring doctors name
• All Insurance cards
• Drivers license or other picture ID
• Coinsurance, copay or self pay cash, check or charge (all major accepted)
